Request Transcripts or Copies of Diploma

You may only order one document type (transcript or diploma) per transaction. If you would like to send your document to different addresses you must complete one transaction for each address.
Your order will be processed within 10 business days. Please do not call the school unless 10 business days have passed since you placed the order.

The Massage & Bodywork Licensing Examination (MBLEx) offered by the Federation of State Massage Therapy Boards requirements (http://www.fsmtb.org/):

  • No diploma or transcript required

The National Certification Board requirements (http://www.ncbtmb.com/):

  • Notarized copy of diploma sent by the applicant
  • Copy of official transcript sent from the school
  • NOTE: The school has permission to send both diploma and transcript directly to the National Board.

The Georgia State Licensure requirements (http://sos.georgia.gov/plb/massage/):

  • Copy of official transcript sent from the school or by the applicant
